Save 10€ / patch test

To ensure a quick turnover for patch tests and to reduce potential errors in the creation of test reports, we have created an interactive PDF order form you can use when sending in products and we are happy to pass on the savings we realize onto you.

The PDF, once downloaded, is an offline tool you can use - no Internet connection is required and no information entered in the form is transferred electronically. However, the form has to be completed on your computer - by checksums that are automatically calculated, we can ensure no errors are made when entering the information into our database.

Discount & Conditions

On each product of a patch test order (simple / photo / tape-stripped) using the PDF order form, you will receive a discount of 10€ if the following conditions are met:

  • The form is completed at your PC
  • A printout of the form is signed & shipped together with the sample(s)
  • The sample(s) are labeled at least with the name as listed in the order form
  • All required information to complete the test(s) & report(s) on the product(s) is available

Should the conditions not be met, the discount is not given on the affected product(s). For example, if we need to contact you because the information on 1 product in an order on 5 products is contradictory, the discount would still be applied to the remaining 4 products.

Using the Order form

With just 3 Steps you are ready to use the order form:

  1. Ensure you have a current version of the free Adobe® Reader® (or Adobe® Acrobat®) installed (Version 8 or higher is required). You can download the most current version free of charge at: http://www.adobe.com/products/acrobat/readstep2.html.
  2. Download our manual (to save on your computer, click right and select "save as"): English Manual / German Manual
  3. Download the order form (to save on your computer, click right and select "save as"): English Order Form / German Order Form

Please read the manual carefully and remember the PDF is "interactive", depending on your input, the form changes and new fields to fill in may appear or be hidden.
